C. United States:

1.  Institutions that provide information and guidance and psychological consultation are too many to list, so we shall only list DBSA (Depression and Bipolar Support Alliance) as one of the most representative organizations.

2.  Institutions that provide a telephone-based depression management program -  MDOCC (Michigan Depression Outreach & Collaborative Care).  MDOCC has experienced mental health social workers as Care Manager who work on behalf of the patient's primary care team and provide them with direct feedback on patient progress. Their Care Managers provide support, education, and monitoring.  They engage through regular phone calls and can provide self-care information sent by mail or e-mail.  MDOCC participants practice self-management skills, have scheduled follow-up calls, and raise any treatment concerns with their Care Manager.   Over 3,700 patients have been enrolled in MDOCC since 2003.  MDOCC reports participating patients have shown significantly better remission rates and increase in work productivity compared to usual care.
